ABSTRACT:
Diclofenac sodium is a widely used nonsteroidal anti-inflammatory drug (NSAID) for relieving pain, inflammation, and improving function in conditions such as arthritis, musculoskeletal disorders, postoperative pain, and dysmenorrhea. Its action is mainly due to non-selective inhibition of COX-1 and COX-2 enzymes, reducing prostaglandin synthesis; however, COX-1 inhibition contributes to gastrointestinal, renal, and cardiovascular adverse effects. Although rapidly absorbed, diclofenac undergoes extensive first-pass metabolism, leading to variable bioavailability and inconsistent therapeutic outcomes. These limitations have prompted the development of alternative dosage forms and advanced delivery systems. This review highlights pharmacological properties, therapeutic uses, safety concerns, and evolving formulations of diclofenac sodium. It also discusses innovative carriers such as liposomes, niosomes, bilosomes, transfersomes, nanoparticles, microneedles, and transdermal systems designed to enhance bioavailability, enable controlled release, and reduce toxicity. Various administration routes are evaluated for their benefits and limitations. Overall, advancements in nanotechnology, targeted delivery, and personalized medicine are expected to improve the safety, efficacy, and clinical utility of diclofenac sodium.
